What is the server field format? Problems connecting

I can connect to my sql server using other tools but not Data Generator.
Connection fails when looking for database…

Is there something specific I need to type in Server field? 

Where do I specify port? Anything else I can do to make it connect?

Your documentation says nothing about the first login screen …

Is your SQL server on an unusual port? have you selected a server from the drop down? Also where is this server hosted? is it hosted SQL as well?
