It seems that you are hitting a known bug SC-9192. It was because the target has two indexes which are functionally the same, but only one of them is in the source with a different name. We think in the engine it would be a rename and therefore ignore it not accounting for the fact there is only 1 of it in the source but two in the target.
Can you please send in an example to confirm it?
