SQL Prompt 9.5.16 has just been released, which should fix this issue for good.

hi @TomW I can confirm its now working fully as expected, reconnecting across multiple restarts of SSMS

Currently I'm experiencing the same issue. SSMS 18.6. Prompt 10.4.2.

I'm also still experiencing the same issue, Tab History won't reconnect any of my open tabs when I start Management Studio. I'm using Management Studio 18.7.1 and SQL Prompt 10.4.7.16599. See my log for SQL Prompt attached.
