I'm not one of the development staff, but I'm sure that this topic is a bit quiet because nobody's willing to divulge the method for plugging in to Management Studio.
Management Studio does have an API that you can hook into, but as far as I'm aware, it's undocumented. This is why Service Pack 2 gave SQL Prompt and Refactor some trouble. Microsoft can change their undocumented API whenever they feel like it!
I'd recommend waiting for Microsoft to publicly document their API for Management Studio, then this sort of thing won't happen to you!
