Toolbar Menu is grayed out?

I have installed SQL Refactor and SQL Prompt.
The Prompt menu works, the refactor menu is greyed out - have I done something stupid ? (Would not be the first time ... )

Are you in this situation described in the article below?

This has the effect of greying out the Refactor menu....
