SQL Prompt Menu is not shown in Visual Studio

I have installed the latest version of SQL Prompt(6.4.1.164).I am sure that the SSMS and VS addin were chosen during installation.But I found the SQL Prompt Menu was not shown in Visual Studio 2010 and Visual Studio 2013,and I can find the menu in SQL Server 2012 management studio.
So can you help me to solve the problem?

The first thing to try is uninstall and reinstall Prompt and just confirm that the check box is ticked in for installing to Visual Studio. Your settings will be preserved between uninstalling and reinstalling.
